3M Issues Fall Protection Recall

In light of reported incidents and potential for misuse, 3M and Capital Safety recently issued a voluntary recall of a product that has been used in the fall protection industry for more than 30 years.

A review of 3M’s original DBI-SALA Lad-Saf Sleeve revealed potential misuse scenarios that raised concern. Misuse can include impeding the product’s braking mechanism through entanglement with cords, lanyards, clothing or other objects; or grasping the sleeve before or during a fall. Misuse also can occur when the sleeve is attached upside down.

3M issued the recall after reviewing its use in the field, including some instances involving serious injury and death.

In issuing the voluntary recall, 3M noted that no federal safety regulator has found the sleeve design defective.

Under terms of the recall, owners and users of the original Lad-Saf sleeves must:

  • Immediately stop using and quarantine all original Lad-Safe sleeves. Click here for affected parts numbers.
  • Contact 3M Customer Service at 1-800-328-6145 (ext. 2012) or email LADSAFNA@mmm.com to discuss replacement of the unit with a next generation sleeve at no cost.

“We apologize for any inconvenience that this situation may cause you, but we are confident that you will be very pleased with the latest generation X2 and X3 Lad-Saf sleeves to keep your workers safe at height,” 3M said in the notice.
