Moisture migration....whewee, don't I know it. Played "stump the contractor" for many months with it in my house. Older home with original landscaping and a very wet summer = high water table beneath the house. Had seepage through a cold joint and many spots where moisture worked through the concrete to impact the carpet as well. Fixed the cold joint and re-worked the landscaping to eliminate the underlying water issues. Not cheap and easily preventable...but at least it was a relatively confined incident (all puns intentional). I'd hate to see what it could do with a fancy finished slab on grade floor in some large commercial space *shudder*.
